Q. At what age am I covered / eligible for Medicare Supplement?
A. Most folks become eligible for Medicare benefits, and Medicare supplement
policies, when they reach 65 years of age. Certain other factors, such as
disability, may qualify a person for Medicare prior to reaching age 65.

Q. How do I submit a Medicare Supplement claim?
A. To service a Medicare Supplement claim, all we need is the following:
A copy of your Medicare Explanation of Benefits form you received from Medicare
("EOMB") with your full name and Bankers policy number clearly written on it.
Most people choose to write it on top, where there isn't a lot of Medicare
preprinted information. That's all we need to process your claim!

Q. I haven't received benefits on my recent Medicare Supplement
claim. What should I do?
A. Our claim service is one of the fastest in the industry. If you haven't heard
from us within two weeks of receiving your EOMB, it is because the claim never
reached us. Just send us a copy of the EOMB and we'll get right to work.

Q. How do I file a Long Term Care or Home Health Care claim?
A. If you are filing a claim for the first time we will need a completed
application for benefits packet (Claim Form)
, which includes information from you, your doctor, and the facility or
provider.
From you:

Forms A & B, completed in full and signed by you or your representative. If
your representative completes and signs the form, please attach a copy of the
appointment.
From your healthcare practitioner (doctor):

Form C, completed in full and signed by a licensed healthcare practitioner.
From the facility, if you are claiming facility benefits (nursing
home, assisted living):

Form D, completed in full by your facility. The facility is asked to attach
documents supporting the application. These are highlighted on the form.
From the provider or agency, if you are claiming home health care
benefits:

Form E, completed in full by your provider. The provider is asked to attach
documents supporting the application. These are highlighted on the form.
After a claim has been approved:

For facility claims - We will need monthly bills from your facility with your
policy number shown.

For home health care claims - We will need regular submission of bills and the
provider daily care sheets showing your policy number.
